Schedule an Appointment in Livonia or Canton
All new patients are requested to arrive 15-20 minutes before their scheduled appointment time to allow the staff to properly prepare their medical file. Downloadable forms are available if you would prefer to complete them at home. Please bring a picture ID and your insurance cards to your first visit.
Please call to schedule an appointment before coming to the office. Urgent and sick office visits can usually be scheduled the same day. If the same day is unavailable, we will make every effort to get you an appointment ASAP.
Visits for non-urgent problems, the management of chronic medical conditions and medication refills can usually be made within a week. If you prefer specific times, it is recommended you call several weeks in advance. We also recommend that you make your next follow-up appointment before you leave the office.
Complete Physical Exams
Please call one to two months in advance for these exams. Physical appointments are usually early-morning appointments and can book up quickly. We also request patients arrive 15-30 minutes before their scheduled appointment. In most cases, there will be health history questionnaires to complete prior to seeing the physician. Patients are also requested to fast 12 hours prior to their appointment, since fasting lab work may be ordered. This means nothing to eat or drink, except for water to keep yourself hydrated. Black coffee is acceptable.
Please call 24 hours in advance if you must cancel an appointment. Calling in advance allows us to open the schedule to other patients waiting to be seen. We reserve the right to charge a $25 fee for “missed/no-show” appointments. Special procedures or complete physical appointments that are not given proper notice to cancel have a $45 “missed/no-show” fee due to the extended time reserved for the appointment.
