Payment Options for Medical Care in Metro Detroit

Payment

Our office requires the payment of deductibles, copays and past-due account balances at the time of service. If you do not have insurance, full payment is expected at time of service.

Our offices accept cash, checks, Visa, MasterCard, Discover and American Express.

Insurance

Our physicians participate in several different insurance plans; however, it is the patients' responsibility to know their insurance coverage and if their physician participates with their plan. Not all of our physicians participate in the same plans and not all plans the insurance company offers are accepted. We recommend you call your insurance company or check the insurance websites to verify that we participate with your plan. Many plans do not offer out-of-network benefits and this may lead to patients being responsible for the full charge.

Our office will submit claims on your behalf for most insurance companies. Please bring your insurance card with you to every visit so we can make sure we have accurate information on file. Please notify us if you have any changes in your name, address, phone number or insurance plans.

Referrals

Managed care/HMO plans require you to have a referral authorization to receive services for diagnostic testing or to see a specialist; you must receive approval from your primary care physician before an authorization can be obtained. It is the patient’s responsibility to request the referral. If you have not seen or discussed this with your primary care physician, you may need to schedule an appointment before an authorization can be requested.

Please keep in mind that our physicians refer to specialists with whom we have working relationships within our hospital network. Although a specialist may participate with your insurance plan, they may not be a member of our referring network, and therefore your physician may deny requested authorizations to specialists in another hospital network.

We request a minimum of three to five days in advance of your appointment to complete the referral process. Same-day referrals will be completed only in cases of emergencies.
